Individuals contact DFTF for referrals to a doctor for a face to face consultation.

DFTF is exclusively a referral, medical records processing, patient screening and patient billing service and does not dispense any medical information or advice. It is not a site that provides prescription medications or controlled substances, is not a pharmacy and has no contracts or agreements with any pharmacy.

Information submitted by the client is stated to be complete, accurate and true. A violation is punishable under federal and state laws and regulations. This is required for the doctor to attempt to provide the optimum treatment and because the treatments prescribed may negatively affect existing conditions. Also, medications prescribed may conflict with medications already being taken.

DFTF does not accept insurance. Terms of payment are detailed in the appropriate section.

All payments are made to a third party collection agency who will disburse the proper amounts due to the doctor for the consultation and to DFTF for its various services.

DFTF consultations are not diagnostic consultations; rather they involve only conditions self-described by the client for which the client is seeking medical advice and treatment. All treatments proposed or prescribed are totally at the discretion of the doctor. The client is responsible for an annual diagnostic physical exam by a doctor

Payments are due prior to the scheduling of the consultations and are non-refundable, subject to the discretion of the DFTF. Clients are responsible for any taxes, tariffs and customs as applicable.

All medical consultations and treatments involve risk. Clients may not get the outcome they seek and, in deed, may even suffer adverse consequences. No guarantees are made regarding the outcome of the consultations or treatments.

Clients with any negative reactions to treatment must immediately notify both the doctor and DFTF.

Clients agree to hold harmless DFTF, its parent K-MAX, LLC, its staff, doctors or anyone associated with DFTF for any consequences of the consultations, the treatments prescribed or the results of taking any medications.

Client is responsible for disclosing to any other medical providers that the client is consulting with DFTF, with detail, and to DFTF any medications taken and not reported on the medical questionnaire (for example, between consultations).

Client should be aware that medications may have to be adjusted because of changes in tolerance levels and also that medications my result in physical and/or psychological dependence.

Medications are the responsibility of the client and are not replaced if misplaced, lost or stolen.

DFTF is bound by HIPPA regulations and all communications, medical or financial are secure. Consent, however, is provided by the client to provide information to others as required for legal reasons, or for the benefit of the client.

All consultations and treatments, whether in person, online by telephone or otherwise are deemed to have taken place in the state where the doctor has a location and is licensed.

The right to provide referrals or not is totally at the discretion of DFTF. No charge, however, is assessed unless a consultation has been scheduled.
